Northwestern University Symptom Tracker Questionnaire

Starting Monday, January 4, 2021, please begin using the Northwestern University Symptom Tracker Questionnaire prior to arrival on campus.

The new Symptom Tracker, developed by Northwestern University in partnership with Northwestern Medicine, will coordinate more fully with the University's data collection systems.

There are three ways to access the new Symptom Tracker:

As was true of our previous symptom monitoring system, the new Symptom Tracker prompts you to answer a daily symptom questionnaire, which you must complete and submit each day prior to attending on-campus work, programming, or activities, including classes.

  • If you are identified as having symptoms that may be associated with COVID-19, you will be issued a virtual red badge and given instructions on how to proceed. Similarly, anyone testing positive for COVID-19 at any of Northwestern’s testing locations will have their badge turned red.
  • Users who identify that they do not have symptoms and who have not tested positive for COVID-19 will be given a virtual green badge, which you must be prepared to display when on campus.

Everyone in the NU community using the Symptom Tracker is expected to provide truthful answers to the symptom questionnaire. If it is determined an NU employee, faculty member or student did not provide truthful answers in Symptom Tracker, individuals are subject to disciplinary actions.
